Seniors Mental Health Program


"Working with seniors and their caregivers to address mental health issues in later life." 

What is the Seniors Mental Health Program?
The goal of the program is to assist seniors to live independently and maintain a level of satisfaction with life in their later years. We provide assistance to seniors suffering from feelings of depression, anxiety, grief, memory loss, Alzheimer Disease or other mental health problems. 

Where are services provided?
There are different areas that services are provided, such as home visits, nursing homes in Kings and Annapolis Counties, and office visits to: AVH Chipman (Kentville), Valley Regional Hospital (Kentville), and Soldiers Memorial Hospital (Middleton). Family physicians as primary health care providers are encouraged to participate in assessment and follow-up.


What are the services provided?
Services are provided by a multidisciplinary team of nursing, psychiatry, psychology and other disciplines as required. Here is a list of services provided as appropriate:

nursing assessment 
psychiatric assessment 
medication review 
care plans 
short term follow-up 
referrals to community resources to enable seniors to remain in their own home environment 
caregiver support and education 
liaison with other community agencies 
community education regarding mental illness and stress management 
consultation to physicians, nursing home staff, hospital personnel and relevant agencies 
Early Stage Memory Loss Education and Caregiver Support Group (Kings County) 

Who can make a referral?
A person can make a self-referral, or can be referred by a family member, physicians, nursing home staff, community agencies or other health care providers.

The business hours for this service are Monday to Friday, 8:00 am am to 4:30 pm, excluding Holidays.
