What is a Community Health Plan?
A Community Health Plan is a report about your community that describes the health of the people in your community, the conditions in which they live and the resources available that can be used to improve health. Community health planning is ongoing. It identifies the communitys health issues and outlines ideas and strategies which, when put in place, will work toward improving the overall health of the community.
It includes:
- Information about our Community Health Board and our District Health Authority.
- Highlights of an assessment of our community (e.g. data, community input and research).
- A description of key health issues and priorities in our community.
- Recommendations for strategies/action for the DHA and local CHBs.

Who Are We?
Volunteer members of the community who are concerned with our community's health out comes. We want to make a difference and give a voice to the unique health concerns of our citizens in Central Kings.
What is "Central Kings"?
Central Kings is located in the beautiful Annapolis Valley of Nova Scotia, Canada. It lies on the edge of the Bay of Fundy. The approximate boundaries of "Central Kings" are Spittal Road and Pelton Mountain Road in the West, Middle Dyke Road and Deep Hollow Road in the East, the Bay of Fundy in the North and the County Line in the South.


The District Health Authority (DHA)
CKCHB is one of five Community Health Boards within the Annapolis Valley District Health Authority (AVDHA). The AVDHA is responsible for providing health services to the Annapolis and Kings Counties in the Annapolis Valley in Nova Scotia Canada. The overseeing body of the Annapolis Valley District Health Authority (AVDHA or District #3) is the AVDHA Board of Directors. This board is made up of twelve volunteers. Two thirds of the volunteers are selected by community Health Boards and one third by the Nova Scotia Minister of Health. To contact the AVDHA please see the links at the bottom of this page.
