*We need volunteers ... have a look at our flyer for information on who we are, what we do and volunteer opportunities!  Without volunteers from our community we cannot continue to fulfill our legislated role and provide funding to community groups/organizations to work on health initiatives.

Our meetings are held the first Tuesday of each month early evening and locations vary. The public is welcome to attend. Contact the CHB Coordinator to confirm the meeting location, see below for contact information.

The focus of our board is to work on issues in our Community Health Plan from a Population Health approach, which means addressing the Social Determinants of Health

 Our other work includes:

"Wellness Funds"

Each year we provide funding to groups/organizations working to address our health plan priorities in our community.  For more information on Wellness Fund criteria and applications click here.

"Active Kids/Healthy Kids Annapolis County"

An initiative to provide funding to groups/organizations who are working to get kids more active in our communities. We are working in partnership with the Municipality of Annapolis County, Town of Middleton Recreation Department, Town of Bridgetown, Family Matters Resource Center and the Department of Health and Wellness-Physical Activity Sport and Recreation.  Check with your local recreation department for more information/application.
