District Cardiovascular Health Coordinator


Reporting to the District Manager Respiratory Services and VRH Medical, the District Cardiovascular Health Coordinator, in collaboration with the interdisciplinary health care team in Annapolis Valley Health (AVH), supports the planning, implementation and evaluation of district programs and services related to cardiovascular health and the integrated stroke program across the continuum. Responsible for liaising with Cardiovascular Health Nova Scotia (CVHNS) and facilitating provincial program activities related to strategic plans for stroke and cardiovascular health within AVH.

The District Cardiovascular Health Coordinator ensures that the principles and practices that guide AVH such as the mission, vision, values, expected behaviours, the leadership philosophy, organizational health, population health, ethics, safety, quality, partnerships and interprofessional collaboration are integrated within the services they provide and through the messages they deliver.


  • Diploma RN or
  • Baccalaureate degree in nursing or
  • Baccalaureate degree in a health related field
  • Registered or eligible for registration in the Province of Nova Scotia or
  • Registration with the applicable regulatory body
  • Minimum three years experience in cardiovascular health and care required
  • Demonstrated ability to collect analyze and interpret data
  • Demonstrated ability in program planning and implementation
  • Demonstrated facilitation skills
  • Current knowledge and demonstrated understanding of the provincial health system, AVH, CVHNS programs, local programs and services
  • Demonstrated collaboration and teamwork skills
  • Demonstrated leadership abilities
  • Ability to work within Microsoft Office suite
  • General computer proficiency including database management, data retrieval, analysis and report writing is required
  • Working knowledge of provincial PAC system and web 1000
  • Working knowledge of Meditech
  • Current knowledge of internal and external partners and a demonstrated ability to network with them to support these provincial programs

PREFERRED: (Considered an Asset)

  • Certification or advanced education in Cardiovascular Health and Neurosciences
  • Ability to communicate in both French & English
