Community Home Visitor


 Reporting to the Public Health Leader, the Community Home Visitor (CHV) worksin collaboration with the Healthy Development Content Lead, and others as appropriate to ensure healthy development programs and services are planned and implemented to address local population health needs.

 The CHV works with at risk families and vulnerable populations during the prenatal, postpartum, and early childhood period.  The CHV implements the Healthy Beginnings Home Visiting program including a strengths based curriculum for at risk families.

 The CHV is part of a collaborative multidisciplinary team approach and works with the Healthy Development team to ensure quality case management.

 The CHV shares a core set of public heath attitudes and values, applies public health core competencies to all of his/her work and is accountable to provide competent, safe and ethical professional practice.

The CHV ensures that the principles and practices that guide the District Health Authority such as the vision, mission, values, strategic directions, ethics, safety, quality, partnerships and inter-professional collaboration are integrated within the work that they do and through the messages they deliver.


  • Diploma in Early Childhood Education or Human Services from an accredited education institution.
  • A valid driver’s license and access to a reliable vehicle.
  • One (1) year recent experience
  • Current CPR and First Aid Certification
  • Computer proficiency with Microsoft Office programs
  • Audio visual equipment and general office equipment
  • Demonstrates the core competencies for public health as outlined in the job analysis. 
  • Demonstrates knowledge in the Health Protection Act and other relevant Regulations, Legislation & Acts, specifically the Child and Family Services Act.
  • Demonstrated ability to communicate effectively, including team building, negotiation, conflict management.
  • Demonstrated ability to work cooperatively and collaboratively in a multi-disciplinary setting.

 PREFERRED (consider an asset):

  • Experience working with high risk families, parents and children.
  • Ability to communicate in both French & English
